Welcome to the Disease Reporting Entry form. This form allows reporters under K.S.A. 65-118 to submit an electronic notifiable disease form or an influenza surveillance report to the Johnson County Department of Health and Environment (JCDHE).
Outbreaks, unusual occurrence of any disease, exotic or newly recognized diseases, and suspect acts of terrorism should be reported within four hours by telephone to KDHE Epidemiology Hotline at 1-877-427-7317. Diseases listed in red on the reportable disease list should be reported within four hours by telephone. During business hours, you may call our JCDHE Disease Containment Hotline at 913-826-1303.
